Beginning January 1, 2026, CMS has respecified the Falls with Major Injury (FMI) measure, introducing updated definitions, revised data sources and important coding implications for home health providers. These changes will directly affect how falls are captured, calculated and reported.
The revised measure incorporates claims and encounter data alongside OASIS submissions, shifting how outcomes are evaluated and increasing the importance of accurate, aligned documentation. Providers must understand how these data sources interact to ensure reporting reflects clinical reality.
Additional updates include key coding changes tied to J1800 and J1900c, which will require careful review of assessment workflows and staff education. Misalignment between clinical documentation and coding may create reporting discrepancies under the new methodology.
This session will break down the updated FMI measure, clarify what has changed and outline practical steps home health agencies can take to align documentation, policies and workflows with the 2026 requirements.
